We cannot perform this work for free, so these extra services do attract charges. Examples include the following:

• Medicals for DVLA forms
• Insurance claim forms
• To Whom it May Concern Letter
• Holiday Cancellation
• Firearm Certificate

We base our fees on the British Medical Association (BMA) suggested scales. Our reception staff will be happy to tell you about them and book an appointment if needed. Please allow one month from the receipt of your request, for completion.

We do not:

• Fill in forms for work which would be better dealt with by occupational health
• Countersign passport forms for UK passports
• Power of Attorney

Note: 

Pre-Payment is required upon receipt of all Non-NHS Requests.

We accept payment by Debit or Credit Card. We no longer accept cash. We have a portable card machine installed at each surgery. Our card machines accept all major credit cards and payment methods including Visa, MasterCard, Amex, Apple Pay and Google Pay. If for any reason you are unable to use our card machines, we will accept a direct bank transfer or a cheque.
