Change of Contact or Personal Details

Please let us know as soon as possible if you change your name, address, email address or telephone number. If your Next of Kin has changed you can use this form to inform us. It is important that we have up to date contact details in case we need to contact you or your Next of Kin.

Please note: If your name has changed due to Marriage or by Deed Poll, can you please provide us with a copy of the appropriate document (requirement of Department of Health).

If you have changed your address, before submitting this form please check to make sure your new address falls within the boundary of your existing or chosen new surgery. You can check that here: See the Boundary Maps for Each Surgery - Coastal Medical Partnership

If your address does not fall within our boundary, you will need to register with a different surgery.
