What is a Care Home Assessment Team?
We are proud to have our own 'Care Home Assessment Team' (CHAT) here at Coastal Medical Partnership.
The CHAT team provides both urgent and planned health support to residents in care. We currently have 20 care homes registered with Coastal Medical Partnership.
What do the CHAT Team do?
Our team visits residents in their care home. This is part of the weekly care home ward round or on an individual basis as needed. They are able to assess, diagnose and treat complex physical and mental health.
The team aims to maximise opportunities to enhance the quality of life of residents. We involve residents in the decisions about their medical care.
How Do You Achieve This?

Treatment and symptom management for sudden illness or conditions
- Falls assessments

Palliative and 'End of Life' care

Advanced care planning

Medication reviews

Long term condition reviews and management

Health education and health promotion advice

Assessment and treatment advice for common mental health concerns including dementia, depression and anxiety

Education and training for care home staff

Urgent blood tests/ECGs
